Index match to different tab

Folks,

I'm sorry for asking this, about a month ago I'd have been able to sort, but alas, I am stooped.

I'm trying to import the content of a cell from another tab, providing 2 cells match across the 2 tabs.

Awful explanation there - apologies.

All I have atm is:

=INDEX(Grade 6!)

I want it to look at the Grade 6 tab which looks like this:

So...if the number in B5 in tab 'Report Tracker' matches with the number in Column D on 'Grade 6' I want the value in Column N of the same row returned.
