index match vlookup

this is blowing my mind, anyone have any ideas it would be very much appreciated

scenario:
i want to find the first instance of a column on that row which matches another cell (cellref) and when found for all rows under that column which equal 1 lookup the value in the left most column from another sheet and pull back its value

cell ref = H3

data:

Country H1 H2 H3 H3 H4 etc.....
Albania 1 1 0 0 1
Algeria 1 0 1 1 1
Angola 0 0 0 1 1


So what it should do is look at the first row to find column value = H3, then under this column where it finds 1 pull back the Country name (eg Algeria), then i want to lookup Algeria against another sheet where it will appear in column A to pull back whatever is in column B

Pressing F4 adds dollar signs when editing a formula. When not editing, F4 repeats last command.

What about where there is more than one '1'?

ive created a sample excel for this which can be found here which kind of shows what i want to do

source tab has the data source
output tab has the desired output

im trying the find the first occurrence of X12 on Source tab, then where the row under that column contains a 1, pull out the Name_## and show all Name_## where this is true
