INDEX & MATCH

Good day,

I'm trying to transfer the phone numbers from column D into column I if the name is on Table 2 list.

Here is my formula "=INDEX($D$3,$D$12, MATCH(F3,$A$3:$B$12,0),MATCH(G3,$A$3:$B$12,0))"

What version of Excel are you using?
