Insert Multiple Options in Dropdown List

I forgot how to do this could someone please help me?

Example:

A1 has the option of state: i.e. VA, MD or DC

A2 then has the option of county for each state, but on the state chosen: i.e. if I choose VA then Fairfax, Loudon and Fredricksburg will come up - not any MD counties.

Then once I choose a county, I was a fee to be input to A3, for example if I choose Fairfax I want A3 to say $20, but If I choose Loudon I want it to populate with $10.

Can anyone explain to me how to do this? Thanks!
