Linking data from seperate sheets based on Name, Date and summing 3 rows together.

Hello, below i inserted two sets of data: SCORES and DATA LOG. im trying to complete the excel sheet to where each person will be able to input their stats and it will auto-populate from the DATA LOG to the SCORES sheet. I need for when someone types their NAME in the DATA LOG sheet, it's recognized in the SCORES sheet along with the SUM of ('DATA LOG'!G:I) inserted to row where the NAME is and the column where the DATE is.

i stayed up til 4 last night trying to solve it and im out of luck.. any help or recommendations is appreciated!

i used index instead to link matching words with the cells but it wont populate to the other cells when i drag it to them. is not dynamic enough to translate...
