Look up last dates with values

Dear Pro Excel Mates,

For my project I need to derive the last recorded Self Grade and SV (Supervisor) Grade.

A little background: employees get evaluated and each row is an entry.

If they evaluate themselves, the Self_Average is recorded (the AVG of all criteria scores) and no Supervisor grades are recorded.
If evaluated by their Supervisor (SV), the self criteria are blank, just as the Self_Average, but all the SV criteria grades are recorded, as well as the SV_Average.
If its an Annual Appraisal, both the Employee and SV will grade.

I want to have a formula that gives me the last Self_Average and one for the last SV_Average, according to date.

I also want a formula that gives me the "Till date" Self_Average (so all AVGs of Self_Average, apologize for ambiguous wording) as well as for the Supervisor "Till date" SV_average.
