Major headache in looking for tool to export data to excel and word

Greeting All,

I have been troubled the last day on finding a tool that would serve as data gathering (one item after next, similar to online survey type) and generate each of these data to a specified cell within a spreadsheet and populate the data in a specified location in word. Appreciate for any advice or suggestion of tool to use. Thank you.
 

Excel Facts

Format cells as date
Select range and press Ctrl+Shift+3 to format cells as date. (Shift 3 is the # sign which sort of looks like a small calendar).
