Manual save no longer working? (sometimes?)

For years I have used the Ctrl+s keyboard shortcut to save a workbook. Sometime in the last week or two, I still get the "Saving..." message in the title bar followed by "Saved", but then if I immediately click on the "X" in the upper right-hand corner, I get the pop-up dialog asking if I want to save it. This doesn't happen with every workbook, but it used to never happen. Curiously, if I click on File | Close, I don't get the message.

Did something change?
 

Excel Facts

Remove leading & trailing spaces
Save as CSV to remove all leading and trailing spaces. It is faster than using TRIM().
Was the file extension of the Excel Workbook .xls, for example, instead of .xlsx, .xlsm, .xlsb? Have you used any macros recently which save the Workbook for you?

Extensions are mostly .xlsx, with some .xlsm. Most use code in my personal add-in, but none that save the workbook.
