Merge text and date cells

I want a cell in my worksheet to read: "for the week ending Dec 9, 2016"

I want this cell to be dynamic so that I can update a cell with the ned date and the cell referenced above will update. I used to use an equation something like ="for the week ending " & B12 assuming B12 is where the date is updated. But that equation returns "for the week ending 42713". How do I format the cell so the date shows and not the number equivalent?
