Merging 3 columns into 2

I have a worksheet with

ID in column
Name in column B & D

I would like to merge in another sheet columns B & D in a single column and with the ID assigned.

You need to change 2 in

=IF(N(E3),INDEX($A$3:$B$4,E3,IF((E2<>0)*(E3<>E4),2,1)),"")

to 3 when the range A3:C4 and A3:A4 and C3:C4 must be merged.
