Merging Column in Excel

Hi, appreciate if someone can assist me on the below.
I would like to merge multiple columns into 1 single column (Ref) under same person (Name).
Does anyone know of way to make this happen?
Any help is greatly appreciated!

Excel Facts

Move date out one month or year
Use =EDATE(A2,1) for one month later. Use EDATE(A2,12) for one year later.

Where you have [Ref], it should be Table1[Ref]
