Missing Pivot Table Toolbar

I have lost the pivot table toolbar and I can not recover it. Along with the toolbar, I have also lost the field list. I have tried to turn it on and off, but it does not return. When I right click on the menu bar and select it, the check mark shows up indicating that I should have it, but it does not show up.

I would greatly appreciate any help as I use pivot tables every day. I do not know how I lost it, but without it I have to add and remove fields with the wizard and the layout button. That is very difficult to work with.

Thanks for the VB code, but I still had no luck. I wonder if I have something currupted.
