Multi Lookup or Index/Match?

I am looking to do a look up on a table but there are 3 pieces of search criteria to be considered.

Weight - Prepopulated data
Duration - selected from a drop down (10-60 in 10 min increments)
Intensity - selected from a drop down (LOW, MED, HIGH)

So the user puts in the two pieces of data and I want it to look at the data and find and return the value from the table.

Each duration increment is split into the 3 levels of intensity

eg

Col G is for 10 mins LOW intensity
Col H is for 10 mins MED intensity
Col I is for 10 mins HIGH intensity
repeat for 20,30,40,50 and 60 (3 cols each)

The values are stored from G4:X20

In Col F you have the lookup weights in 10kg increments.

F4:F20 (40kg to 200kg)

So if i used the values

Weight = 60KG
Duration = 10
Intensity = MED

It would look down Col F to F6 (60) and along to H6 where it would return the value stored there. Based on COL H being Duration 10 (from H2) and Intensity MED (from H3)

If anyone can make sense of that can you help me out please?

I tried this formula from an old, similar thread, but it returned the wrong value:

I have refined the weight increments so there is more rows of data.

This formula only uses the Weight value (I3) and Duration value (I4) stored on another sheet. I need to include the 3rd value Intensity (I5).

Please help.
