Multiple Data Observations to Columns

Hi all;

I have a table of ~5,000 rows with 5 or 6 data items per unique index value, and need to 'distribute' the data items into columns so I end up with a single row for each unique index value. (database terms = normalizing). Can't figure out how in Excel, can you help?

Ex:
hospital 1 has 5 results, hospital 2 has 7 results, hospital 3 has 3 results, all in the same row. Need to end up with 1 row per hospital and a column for each observation (ED1, ED2, etc). See below and thanks!
