Multiple If statement

I have a spreadsheet with 2 criteria:

1) If less than 5
2) If greater than or equal to 12

Column C is the store number
Column D is the question number(2 for each store)
Column E is the criteria number

Example of spreasheet below:

Column C Column D Column E Column F(formula)
123 1 4 Yes(question 2 is =>12, and 1 <5
123 2 12
124 1 5 No quest 2 is less than 12
124 2 11


Any help with an if statement here?

Using this formula in F1, you can just drag the formula down and it should work.
