Multiple Lookups

Good Afternoon all,

I have 2 spreadsheet in one work book. The first spreadsheet contains a code and the second spreadsheed contains a provider number and a recommended fee. The issue I have is that not all codes have a provider number.

How can I do a match from the procedure code from the first spreadsheet to the correct recommened fee? I tried to do 3 VLOOKUP, but that did not work....


Any help will be greatly appreciated.
