Perform VLOOKUP and Return ALL Matches to One Cell

I have a lengthy spreadsheet displaying medications for a group of patients. Each patient takes between 1-5 medications per day at varied intervals (some at 8AM, 12PM and 6PM, some at 8AM, 6PM, bedtime, etc.)

I need a way to create a "cheat sheet" for the staff. It would show, at a glance, which meds are to be given at which time slots for each patient.

To do this, I need to perform a LOOKUP of the patient's name, and then return multiple values from another sheet (one for each medication that they take). Here's the kicker, I want to display the medications in ONE cell next to the patient's name. For example, A2 would contain "Jim Smith" and B2 would contain "Zoloft, Clonadine, Concerta".

I've looked at the posts about INDEX, etc to return multiple results, but I'm having a hard time getting the results to appear in a single cell.
