'Phantom' custom toolbars

I have 4 workbooks that used to contain custom toolbers but now use command buttons instead.
Despite having followed the Help instructions and deleted all 4 toolbars with Tools|Customise|Toolbars and re-created the Excel11.xlb file they still (intermittently!) re-appear.
When I examine the macros attached to them they are reported as belonging to a workbook which has long since been deleted! I have tried re-assigning current macros to them and removing them again but they still re-appear, unchanged.
Can anybody come up with any suggestions?

Create a chart in one keystroke
Select the data and press Alt+F1 to insert a default chart. You can change the default chart to any chart type
Are the toolbars attached to the workbook? Right click any toolbar, choose Customize and click Attach. If there are any toolbars in the list on the right select and delete them.
