Pivot table from a table with multiple stacked column headings

I have a table which is a similar format to this:

United States England
Georgia Alaska Norfolk Devon
Name

Alf 1 4 3 6
Bert 2 4 4 7
Charlie 3 3 3 3

Is it possible to do a pivot that would give the a total by person for the United States and England rather than the seperate states/counties without moving the US and England so that it is below the states/counties?
