Pivot table level

Hi. I have a pivot table that somehow I've managed to accidentally change (screen shot 1) but can't figure out how to change back. I have a previous version of the workbook.
My problem is with the Exit Date Column. I wanting to show Month/ Day / Year (as per screen shot 2) but just don't know how to do and not really sure what I did to make it change. Any help greatly appreciated

Excel Facts

Copy formula down without changing references
If you have =SUM(F2:F49) in F50; type Alt+' in F51 to copy =SUM(F2:F49) to F51, leaving the formula in edit mode. Change SUM to COUNT.
I'd say you either accidentally hit the [Show Formulas] button in the Formulas tab, or hit the keyboard combination [Alt]MH. Just go to the Formulas tab and click the [Show Formulas] button or use [Alt]MH to toggle it off.

Don't understand why (blank) is showing in both samples unless that's the value you've given empty cells. If you did that to be able to filter out blank cells, right click any cell in the Pivot Table and select PivotTable Options... and check the box to the right of For empty cells show: and clear anything in the field to the right.
