pivot vlookup

Hello,
I have a very large dataset that shows unique identifiers against codes recorded against that identifier. Because there is so much data I can only present the data in a pivot like this:

On a separate worksheet I am trying to do a vlookup to bring through the data like the below:

UI CODE 1 CODE 2 CODE 3
94517 E87.2 J96.90 J98.8

But I cant get it to work?
Is there a way for me to do this?
Any help appreciated
