PLEASE HELP!!! use a button to choose a different sheet bas

I have a couple of comboboxs on the first sheet, the next sheet the user has to enter data and click a button. I want the button to send them to another sheet based on the choice/s that the user made in the two comboboxs.

Some of the sheets have the same name as the options in the combobox but others are different. Like two options in combobox2 (Housekeeping/Custodial & Plant Operations) need to go to the "Facilities" sheet. Whereas two options from combobox1 (HC Acute & HC Services) need to be directed to the "HC Division" sheet and possibly another sheet from combobox2...Like if the user selected "HC Acute" in combobox1 and then "Plant Operations" in combobox2 the user would need to complete the "HC Division" Sheet as well as the "Facilities" sheet.
Can you help????
