Populating summary table

I want to be able to look up in the Risk assessment table which is about 250 lines and includes about 30 Group Hazards. I want to be able to create an automatic summary table when a comment is added after the "potential consequence" the entry in the comments will always lead to an assessment that is populated manually. as you can see 1 group of hazard can have several Origins with specific consequences. I have also attached my Summary sheet at the end

Welcome to the Board!

If you fill in all of the "Type or group of hazard" rows, you can probably summarize with a pivot table. Just note that you'll need to do something to consolidate your current rows 1 & 2 as a pivot table will only see the first row as the header and all others will be seen as data.
