Return Row based on input field and mark another field as reviewed

Hello, I have a large set of data. Each row is data for an employee that needs to be reviewed.
Following is what I want to do:
Have a form with an input field based on ROLEUSER field.
They enter their ROLEUSER name and the form returns the data from their row.
I want a user to be able to search for their name(ROLEUSER) and only see that row of data.
They would amend the data with a Yes or No that all is correct or not.
If no, they would add a comment.
Since these are employees, it would be great to be able to pull the email/or userid and insert that also to confirm that the person reviewing is the same as the person being reviewed.
Is all of this possible?
I couldn't use the XL2BB so hopefully my small sample will be sufficient. (I have more columns and many more rows in my actual data.)

Excel Facts

Add Bullets to Range
Select range. Press Ctrl+1. On Number tab, choose Custom. Type Alt+7 then space then @ sign (using 7 on numeric keypad)
