Returning 5 most recent results based on conditions

Guys,

I've managed to get this kind of working (with your help)... but, I need to add more bits...

I need to add reference number and remarks from the audit! AHHHHHHHHHHHH!!!!!!!

I have the problem that sometimes 3 or 4 audits are carried out on the same day, their only difference is the ref number and 'usually' comments...

How do I make this work now?
