Send email if Open Issue is Overdue

Hi all,

I have an excel table which lists Customer Issues. The update depends on people remembering to access it and close out their issues.

In Column A there is a sequential record number.
In Column B there is the Customer name.
In Column C there is the email address of the allocated co-ordinator.
In Column D there is a running count of Days Open.

What I need/would like to do is,
If the Days open in any row of Column D exceed 15 days, the Record Number, and Customer name that is in the same row, is copied into an email, and sent to the email address of the allocated co-ordinator for that concern, with the title "Overdue Concern requires closure".

We also use Lotus Notes for our emails.

Is this possible?
I am still new to this sort of coding, and I have looked at lots of threads on here. There are lots of threads that are close to what I want, but not quite what I need.

If you need more info, please feel free to ask.
Any help is greatly appreciated.

Surely there is someone who can help me, or at least point me in the right direction. By the amount of hits this post is getting, it looks like a solution could help more than just me.
