Sheet not functioning properly

I have a workbook that is supposed to open a form window and keep the workbook window hidden, which it did for a time. First it issue was after opening and closing once, the next time you open the workbook it would open both windows. Now it does it every time and I dont have the complete understanding necessary to troubleshoot why since this was pieced together from research and help from here.
