Template Wizard Error Message (Same Name...)

I'm using the Template Wizard (Excel 2003) with data tracking and just setup the form along with the database but am getting an error that reads;

"A document with the same name "Per Diam" is already open".

This happens right after starting the Wizard and after identifying the .xlt file to connect with.

Any ideas what I might be doing wrong? Help would be greatly appreciated.

I found the answer on another site. Press Ctrl + Alt + Delete to open the "Task Manager" and it showed two of the same files running at the same time. Don't know why, but that resolved the issue once they were closed and returned to Excel.
