userform duplication trouble

I have two command buttons which opens a userform that asks the user to 'open document' or 'cancel'

I seem to be duplicating alot of userforms.

Is it possible to to have both of these buttons open only the one userform and when I click 'open document', it should detect which document to open depending on which button I pressed on the worksheet?
