Using a VBA userform

Hello, i would like assistance with a userform. I am creating a patient list and i have a column where i have created a userform to contain all of the diagnosis codes for the patient. I would like to use this same userform for each row, which will be a new patient, rows 5-999... However, when i select a new row and the userform opens, after making a selection, it only applies to the first row, and nothing goes into the row selected. Can you assist me with this task? I already have it set up to allow me to multiselect diagnosis codes, however, if patient 450 requires different diagnosis codes, i would like for them to only be inserted in that specific row for that patient. please help! thank you!

It's a bit difficult to help you if we can't see your code to see where it might be going wrong.
