Using forms to Create Database

I would like to make a VB form that will open when the workbook is loaded that has fields for entry (such as name or address) and upon hitting a submit button at the bottom will input this data into the workbook, and then will reopen the form. The second forms inputs should be recorded on the second row of the same worksheet.

Sorry for such a long and elaborate question but I am new to VBA and would like examples for applications I have right now as well as "learning by doing".
