variable input boxes dependant on user choice

Not sure where to start with this as not had need of it before.

I need a user form to generate the appropriate number of user input boxes specific to the user request. At the moment we just use a normal spreadsheet but want to create a form

e.g. A box asks the user how many accounts he needs which he responds to.

If his response is 4 then i need the form to generate 4 'sort code' and 4 separate 'account number' input boxes for the user to complete

If his initial response is 2 then i need it to populate 2 sets of input boxes

Help or pointers greatly appreciated

There is a drop-down next to the AutoSum symbol. Open the drop-down to choose AVERAGE, COUNT, MAX, or MIN
Why not just create a form containing the maximum # of 'sort code' and 'account number' that a user might enter. Hide them all during developmnent and then make visible as many as user says they need? This is an alternative to dynamically creating and positioning controls which gets a little more complicated.
