VBA Help - Creating templates, naming them, and filtering to project type

Hello,

Can anyone help me figure out how to setup VBA Macro to create multiple named templates and place into folders of the matching Project.

See Table below. I want to see if it is possible for it to create templates using Form Type A, name it 'Title_1' , then place it into folder for Project Red.

I have a list excel with columns of, File Title, Form type (different templates ex. Template A or Template B), and Project.
