VBA to Search Data Table Find a cell And replace info with Info from another cell from another sheet

I have done this in the past, but I have not been in excel in almost a year and I cannot even remember where to begin. I have created a form that stores info in single rows in excel. The issue I am facing is that I have 2 cells that will need to be filled out after the employees signs the form. So what I am trying to do is :

Search the value of sheet 4 cell g33 against a table on sheet 21 (EAF_Report_Table[[#all],[Error '#]])

When G33 returns a find on the row I then want to replace the information in 2 cells

Add value1 is sheet 4 cell D15 : Adding to EAF_Report_Table[[#all],[Date Discussed]]

Add value2 is sheet 4 cell C24 : Adding to EAF_Report_Table[[#all],[Supervisors Planned Followup (What and When)]]

Please help, and thank you.
