Vlookup failed................. solve the mystery

I have data as following :-

Roll# Name English Math Science
215 ABC 99 75 85
100 DEF 55 65 75
300 XYZ 85 95 99

I want to apply vlookup in the following by matching two keys simultenously but vlookup is faild to provide the required result.
Dear All,

I have a set of information like this:-

Roll# English Math Science
100 Vlookup Vlookup Vlookup
300 Vlookup Vlookup Vlookup
215 Vlookup Vlookup Vlookup

It would be appricieated if any body can help me.

I want to get the result by using this formula of Vlookup:-

FOR ENGLISH:
=VLOOKUP(Rol&English,<Range>,3,false)
FOR MATH:
=VLOOKUP(Rol&Math,<Range>,4,false)
FOR SCIENCE:
=VLOOKUP(Rol&Science,<Range>,5,false)

Problem is that how can I search by concotenate two cells and then find it a range which has matching keys in a separate form.

Plz help

Dear experts I need your help. again I am stuck with this formula.

My requirement is to fill data in the range-2 by considering 2 column given left in range-2. Plz help me how can I use SUMPRODUCT/VLOOKUP/MATCH/INDEX to get the desired result.
