Vlookup to return the highest value and descending

Hello,
I've created a spreadsheet for Trending for a month's worth of data.
I have each "week" in a different tab.
Is there anyway write a formula for a V-lookup to return the highest value and descending for the top 4?

example:
The data would be pulled from a sheet that has initials going across in the rows. Each column is labeled with a different "Title".
I need the formula to return the value for the initials, AND return the title in which it was found.

Is this possible??

And you want to return the 4 largest number values and the corresponding column headers?

95 - LS
63 - JK
63 - DD
62 - BN

I'm having problems actually pasting on the image of my spreadsheet.

Column A lists all the initals.
Row 1 lists all the "titles" example: Wrong Pt, Wrong Drug, Wrong Quantity.
A2 would have "MIS" and all of Row 2 would show the total number under each category for those initials.
