I have a document which lists patient id numbers, equipment ID, and diagnostic codes. In the document I am working on I want to return the correct diagnostic code but the correct code depends on the equipment ID. How can I accomplish this with formulas? If I use a vlookup it will return the first code found, in this example M62.81. However, in this case I need the code for RNT Nebulizer - J44.9. Both documents have both the Patient ID field and the Item ID field.

Any help would be appreciated!

Example

Patient ID
Item ID
Diagnostic Code
E0939
RNT SOLACE MATRS
M62.81
E0939
RNT NEBULIZER
J44.9
