.xll add-in opens as a spreadsheet

Running Office XP in a Citrix MF XPe environment.

Some users have opened Excel and see one of our .xll add-ins opened as a spreadsheet. On the title bar it shows that the open document is "scxadd32.xll [read only]" and the spreadsheet is alot extended ASCII characters. The first line has a cell that states "This program cannot be run in DOS mode" preceeded by a bunch of extended ASCII characters. You can see the same type of thing if you open the .xll file in a hex editor.

This is a problem but what makes it harder to troubleshoot is that we can't reproduce it. It happens sometimes and then goes away.

I've been all over google and the microsoft websites but can't find anything about this. Has anyone ever seen anything like this? Any ideas of what might cause it?
