I need the two drop down boxes at the top to change the information in the form below. A user should be able to select their name and the month they're on, then put the information in the form below. Once the information is in it will save it. I'm currently using Google Sheets but I can convert from Excel to Sheets w/o issue I just need a starting point. Here is a screenshot of what I have.
