Detect date/time collisions with multiple criteria

I have data that I am trying to determine where date/time overlap (TRUE/FALSE) works but only when certain criteria are met.
The collision column is what I'd like to do via formula or macro...

I'm able to find collisions using SUMPRODUCT (found plenty of examples) until I start adding criteria, but that won't work since I need to find date/time collisions only when address and identifier are the same.
