I want to return the column headings for each x in a record (row) I can find the first using index and match but not subsequent column headings.

Secret function! Use =DATEDIF(A2,B2,"Y")&" years"&=DATEDIF(A2,B2,"YM")&" months"&=DATEDIF(A2,B2,"MD")&" days"
VBA or formula?

A formula would simply be (=If(A3="x",A$1,""), which you'd paste in A3 and then drag to the following columns

For VBA
Code:
For each cell in ThisWorkbook.Sheets(1).Range("A3:E3")

    If cell.Value = "x" Then MsgBox "Your header in " & cell.Address & " is " & ThisWorkbook.Sheets(1).Cells(1, cell.Column).Value

Next cell
