I have a spreadsheet which has first and last names in columns B, E. H, L & O and phone #s in columns C, F, J, M and P.

I need formula in cell C4 such that when I type/fill in first and last name in cell B4, the formula brings the corresponding phone # to cell C4.

Excel Facts

Last used cell?
Press Ctrl+End to move to what Excel thinks is the last used cell.
First, create a true database where each line or record has all the information. And then use VLOOKUP

Having multiple columns of names and multiple columns for phone numbers means that you will have to do a nested if statement to check each set of names/Phone numbers until you get a return.

I'm just guessing from your relative cell positions, but would this work?
=if(b4=e4,f4,if(b4=h4,j4,if(b4=l4,m4,if(b4=o4,p4,"Not Found"))))
This will find the corresponding phone (F,J,M,P) of the corresponding name (E,H,L,O)
Name must match, Johnie Smith won't match John Smith
