Auto-generate sequential numbers

I am working on a project where I need to have item numbers auto generate into a table, once the user fills in a specifc cell in the title with a number (eg. Site Review No. 2) and the item list would auto generate 2.1, 2.2, 2.3 in the below table. Is this possible? I have searched online and can't find any help.
