Index Match multiple criteria

I am trying to create an index match formula based on multiple criteria, as per this article:

In particular, I would like to use the NON-ARRAY formula, as stated in the article, ie:

This is my code but it errors on the last line:

Excel Facts

Copy a format multiple times
Select a formatted range. Double-click the Format Painter (left side of Home tab). You can paste formatting multiple times. Esc to stop
Try this way
