Multi-variable vlookup formula

Hi,

I'm struggling to find a formula to find the respective bonus (under criteria below) from the lookup values if I'm given state, title, and amount below. If the title state under lookup values starts in cell A2, what formula could i use to find the respective bonus if given state, title and amount range? Any help would be greatly appreciated.

Lookup Values Criteria
State Title Amount Bonus State New York
New York Banker $0-$9,999 $100 Title Banker
New York Banker $10,000-$19,999 $300 Amount $15,000
New York Banker $20,000+ $600 Bonus ?
Pennsylvania Broker $0-$9,999 $400
Pennsylvania Broker $10,000-$19,999 $800
Pennsylvania Broker $20,000+ $1,000
