Pivot Table Deliverable Count by Date

I have a table that I track deliverable in. In the table each deliverable is a column and I record the date it was received for each project. I was trying to create a pivot table where the rows were year/month, columns were the deliverable's and the body showed the count for each deliverable for each year/month. (see example). Any idea on how to make this work?
