Multiple lookup criteria

Hello there.

I need help with a lookup matter.
My efforts to use INDEX and MATCH have not worked.
A system generated report is received is in columns and looks like this:

I need to put each Ticket Number, with the different headings in a row so each row is unique.

NOTE: I have only used TWO of the Ticket numbers - there could be a hundred.

Your post is virtually impossible to distinguish columns as presented. Here is information from the forum rules on how to post:

N.B.
- With any of the Add-Ins with recent versions of Windows, you may need to "unblock" the file. In Windows Explorer, find the file, right-click, Properties, General tab, tick the 'Unblock' box down near the bottom.
- If you end up with two small squares in your Reply window with any of the HTML makers, close any open Windows Explorer windows and try again.
