    If you have three different dates in each row, the following seems to work OK: -

    =TEXT(MAX(H3:J3), "dd/mm/yyyy") &" "&ADDRESS(ROW(),MATCH(MAX(H3:J3),H3:J3,0)+COLUMN(),4)

    Autofill down for each of the rows you have data in.
