Lookup of two variables in a table

I've got two variables, cust# and store #s. The cust# are unique, but the store # is not. I need to lookup the cust and store numbers and return a value I will assign in a seperate table. Ex., Cust 3, Store 1 is Jack NY, Cust 3 STore 2 is Jack Boston and Cust 2, Store 1 is Ted Washington. What formula or lookups can I use?
