Form Template/Data Tracking issues

I have a checklist form that I am developing. It has about 90-100 yes/no questions in 10 different categories. For each category I have a formula that calculates the % of positive responses. I have generated a summary row at the bottom of the sheet to list a few text entries by cell reference, the numerical values from the % of positive reponses (by cell reference), and an overall average of the % of positive responses. As I am defining the cells and field names I get a Type mismatch error (13) when entering my first numeric cell reference. I have tried just transfering the text values and that is working, but I need to be able to transfer the text cells as well as the numeric. Any ideas?

FYI,

When assigning the reference cells on a blank form a #DIV/0 will cause the Type Mismatch (error 13). I resolved the issue by changing the formulas from

=G12/(G12+H12)*100 [results in #DIV/0]

to

=G12/(G12+H12+0.0001)*100 [results in 0]

This allows formatting the innitial template blank rather than having to enter bogus data.
